Repeat Wound in Pilonidal operated area.

Hello Doctor, I am a final-year medical student seeking your advice regarding a pilonidal scar breakdown. I have a history of pilonidal sinus with two prior surgeries: ​Limberg flap cover (4 years ago). ​Excision with healing by secondary intention (2 years ago). ​..Current Status & Investigation: Recently, I developed intense localized pain accompanied by slight serosanguinous discharge. I consulted a general surgeon and underwent a USG of the local region to rule out a recurrent fistulous tract. The USG came back negative for a deep collection or tract, noting only cutaneous and subcutaneous edema. ​..The Issue: The superficial wound seems to heal, but it repeatedly breaks down, bleeds, and causes pain whenever the sacrococcygeal area is subjected to mechanical stress (prolonged sitting for my exam prep). I am currently experiencing another active flare-up of this superficial tearing. ​Given the negative USG and the mechanical nature of the breakdown, I would appreciate your advice.

Inquiry Regarding Running & Post Surgery

Hello Doctors, my name is Brendon. I am seeking your advice regarding my physical activity and post-op recovery. I underwent an emergency laparotomy splenectomy to remove my ruptured spleen following an accident last year. Before the accident, I was an active runner participating in 5km and 10km races.  Six months after the surgery, with the clearance of my surgeons, I began running again. I started with shorter distances, 2.5km & 5km races, initially at a slower pace of around 8 min/km. Currently, i have increased my pace to around 6:30 to 7 min/km. I have not experienced any pain or discomfort at my incision site and I have not done any recent checkups. I would like to inquire if it would be safe for me to participate in a 10km race in July 2026. Running a half and full marathon is one of my life goals! My concern is the risk of developing a hernia or other complications, if any. I would greatly appreciate your professional opinion/guidance regarding my status. Regards, Brendon.

Post Surgery Recovery

Hello Iam at week 5 after hemmorhoidectomy, anal dilation and spincterctomy. My surgeon stopped the laxative after day 5-6 I never had any hard stools.. just had dry tip which caused pain at the start then the rest stool was normal and is normal now and no pain.. Just when i sit for too long i have pain and... It itches when i walk... And sometimes pricking sensation How many days to be back to normal?

Fistula grade III ,over 1 year

MRI of perineum done with T1 weighted, T2 weighted & Fat suppressed sequence in axial, coronal and sagittal planes of pelvic outlet. Post contrast study done. FINDINGS: A thin hyperintense track with surrounding inflammatory changes is seen originating in the natal cleft in right paramedian location extending for length of 3.5 cms traversing the transphincteric plane with internal opening at 6 o clock. The internal opening is around 1.9 cms from anal verge. No supra-levator extension of the track. No evidence of collection/abscess seen in the present study. The pelvic organs appear normal. No obvious significant pelvic lymphadenopathy. Rest of the pelvic organs appear normal. Visualized bones appear normal. IMPRESSION:- This examination reveals trans-sphincteric fistula with internal opening at 6' o clock position (Grade III fistula). This report is six months old. I have no pain, but sometimes pus comes out, white or watery, without other problems. Please help me i dont want surgery.

However , now a days many Laser procedure are there. Why non laser treatment ? I got my MRI yesterday of a fistulogram and that shows I have fistula..however last month I did one and that showed 1.5 cm long 3mm in width.. now it's 3.8 cm long and 2 mm width ...also attached picture Please let me know if this is simple one or complex or is it moving to scortum region?? I am having little discomfort near inguinal area since few days bot sure because of fistula or other reasons...

Gallstone problem

Sir/Mam, My USG report shows 13.3mm gallstones. I have no problem/ symptom due to this gallstone till now. However, Dr advised me for surgery and prescribed no medicine in this regard. I have a confusion in this regard that, since there is no problem, why Dr advises to undergo surgery to remove the gallbladder. So, kindly advise me if there is any other treatment other than surgery or not ? Or can I defer the surgery ?
