Tooth and Mouth Problems

Please help me in removing these black stains from my teeth. This is dirt. I had done dirt removal process 2 years ago and it has come back. Dont want to use the same method again as my teeth is fragile.

What are the charges of filling and cleaning and which is the best treatment in filling can any one suggest me so that i can make my mind

My dad had burning in mouth 1 year back by taking medicines it cleared up and he had white patches on both sides but we didn't went to dentist since then but after hearing about leukoplakia on YouTube I'm concerned and dentist that time told to do biopsy to be on safer side he never chewed and never took ciggarette so should we do biopsy or not?
