Hi First of all ur child has gone througha lot right from the birth. First preterm birth with low birth weight  then surgery and now feeding issues..  Usually infant have poorly developed tone of the valve which holds the food in stomach and prevents vomiting. As the child grows the tone starts improving starting 6months and vomiting issues resolve..
Next Steps
Since ur baby has been seen by many doctor's and various medicactions are already tried u need to see a specialist(Pediatric Gastroenterologist)  for evaluation of cause.
Health Tips
Here are few tips which can help 1. Raise the head end of baby's bed by 30 degree. 2. Give good 15-20min of burping post feeds. 3. Increase the calorie content of feed and make it more thicker. 4. Give Small frequent feeds rather than large feeds.
