Hello Doctor,
I am a 25-year-old female. My recent blood test shows Vitamin B12 = 225.1 pg/mL (reference range: 197–771 pg/mL). I also have a Vitamin D deficiency as per my report.
I would like to know:
Is this B12 level considered low or borderline?
What treatment or supplementation is recommended for B12 and Vitamin D?
Should I consult a general physician or a specialist?
Thank you

First of all we should focus on symptoms of vitamin B12 deficiency like tingling, numbness, parasthesia etc along motor difficulties. If they are there and value is low or low normal then vitamin B12 should be taken by IV and IM routes and it is compulsory. Without any symptoms it’s your choice to eat supplements or you can simply have a vitamin B12 rich diet which is predominantly there in animal origin of food. If you have low vitamin D it should be supplemented.
Do also remember to get your CBC, Thyroid function tests HbA1C and do provide your autoimmune and dietary history to your consulting doctor
Treatment of vitamin D consists of weekly sachets of 60KU cholecalciferol for 6 weeks and for vitamin B12 you need daily injection for 7 days(IV), then once per week(IM)for 4 weeks, then once per per month for 12 months(IM)

Interpretation of your reports
Vitamin B12 – 225.1 pg/mL
Although this value falls within the laboratory reference range, it is considered low-normal / borderline.
Levels <200 pg/mL → deficiency
Levels 200–300 pg/mL → borderline (can still cause symptoms like fatigue, hair fall, tingling, poor concentration)
Many clinicians aim for a level >300–400 pg/mL, especially in young females.
Vitamin D
Since your report mentions deficiency, treatment is recommended regardless of symptoms.
Recommended treatment
Vitamin B12
If no neurological symptoms:
Oral methylcobalamin 1500 mcg daily for 6–8 weeks, then weekly or monthly maintenance
If symptoms like tingling, numbness, or severe fatigue:
Injectable methylcobalamin may be advised (after clinical evaluation)
Vitamin D
Cholecalciferol (Vitamin D3) 60,000 IU once weekly for 8–12 weeks
Followed by maintenance: 1,000–2,000 IU daily
Take with meals (preferably containing fat) for better absorption
Ensure adequate dietary calcium and sunlight exposure
