Sudden fever , cold, and muscle pain
My friend has been experiencing sudden chills from last night before that a week ago he has been having fever dry cough and muscle ache . And after eating even normal diet he has been experiencing upset stomach and when asked about burning sensation while peeing he said it happens sometimes . What should be done

From your above mentioned symptoms your friend is probably having combined upper respiratory tract infection with urinary tract infection. Since the duration is 5 days and having muscle pain it may be due to viral cause

-need to do blood tests -CBC,RFT,urine analysis,chest xray,ESR,CRP and urine culture -tab azithromycin twice a day for 5 days -tab paracetamol 500 mg twice a day for 5 days -tab pan 40mg before food twice a day for 5 days - drink water about 1.5 to 2 litres per day- drink hot water -salt water

Dear Sir/ Maam, The symptoms you have explained are clear but the progression of each symptom seem a little vague. You should state which symptom occurred first and what the last. But from the preliminary data, I can see there’s a Urinary Tract infection in your friend. Though, the history of chills and rigor does appear like it’s a Proximal Urinary tract infection. Most probably Pyelonephritis or Proximal ureter intection.
Next Steps
I should recommend you to have some blood tests that I can only recommend with a consultation. In between the wait for the test results to come I would start an Empirical Antibiotic with some other medications that will alleviate the symptoms for sure.

URTI with UTI infection.your friends needs to be evaluate first properly. Go for cbc and basic blood culture to ruled out any specific bacteria and viral infection.consult me on practo for further management and evaluation.

It's a mixed infection of the respiratory , gastric and urinary  tract. He needs immediate care

Your friend may be having upper respiratory tract infection and Urinary tract infection as well.. Get cbc, urine microscopic examination done and share results.Give him tab augmentin 625mg one morning and one evening for 5 days. Tab mondeslor one daily for 5 days, in evening. Syp grilinctus dx, 5m.l. every 8hours for 5 days. Syp citralka,  5m.l. every 8 hours for 5 to 7 days.

Kindly done all the routines with urine :r/m urine: c/s For burning sensation in urine drink plenty of water/fluids Is the patient experiencing any other symptoms like lower abdominal pain ,urgency ,hesitancy if yes then it could due to uti For that you can give Disodium hydrogen citrate alkalizer mixed 3 tbs in water drink three times a day For upset stomach avoid triggering foods Tab pantop 40 mg od before breakfast
