If someone don't know if there wpw with afib or not by mistake someone take bisoprolol or propanol in that case with wpw with afib there is safe to take till the time we reach hospital

Hello, A single dose by mistake should not cause major problem. But, generally bisoprolol and propranolol are not advised in this condition. These medicines should anyways be not started without a physician’s advice. You can opt for further consultation for evaluation and detailed discussion.

There is an approach to managing this problem. We call it ‘Pill in Pocket’ approach. Basically as soon as the patient becomes aware that they have gone back into a paroxysmal tachycardia (could be SVT or re-entry or even AF), they take the recommended heart tablet (which for a lot of patients is a Betablocker or another different medication like Flecanide) to see if the episode settles. If despite taking the tablet and waiting for an hour or so, if the paroxysmal tachycardia hasn’t settled or if they develop chest pains/breathlessness/feel faint, then they go to the hospital. The vast majority of patients simply manage with the ‘Pill in Pocket’ approach. If they are needing to use the tablet frequently (more than once a week) then they go through consultation with the cardiologist again. This approach saves patients unnecessarily needing to go to hospitals. ***The most important thing is such a ‘Pill in Pocket’ approach Must always be recommended by a Doctor*** All the best. Please do not take such medicines by yourself without having a proper diagnosis. WPW syndrome, Atrial Fibrillation etc are conditions that are not self assesed. All palpitations cannot be controlled by propranolol. You have to take expert advice physically.

If a patient has WPW syndrome with atrial fibrillation, medicines like bisoprolol or propranolol are NOT safe. These drugs block the AV node and can increase conduction through the accessory pathway, which may lead to very fast ventricular rates or ventricular fibrillation. If the diagnosis of WPW is not known, taking a beta-blocker in a fast irregular rhythm can be dangerous.

• If there is sudden onset of fast, irregular heartbeat, do NOT self-medicate • Seek emergency medical care immediately • If already diagnosed with WPW, inform doctors clearly on arrival • Safe acute treatments in hospital include procainamide or electrical cardioversion, depending on stability • Long-term definitive treatment is radiofrequency ablation, which is curative in most cases
