From last three days I am getting hives and itchy skin in the evening. I visited physician and have taken citirizen I am under medication now. Inspite of taking tablet the rashes are appearing. I have thyroid, in the last Dec my readings are normal. When I did CBC the last Jul my readings were also normal. I read on internet that if the rashes appear at evening it is a serious condition of some hidden infection. Please suggest

Repeat cbp, get blood sugar, stool for ova and cysts, done, share results.
Continue taking tab cetrizine, take tab monteleukast 10mg in evening for 5 to 7 days, tab prednisolone 20mg one daily for 5 days.
Avoid scratching by nails as it can lead to secondary infection and boils.
You have:
• Hives for 3 days
• Worse in the evening
• Itching
• Taking cetirizine but still getting rashes
• Known thyroid disorder (currently controlled)
• Normal CBC previously
First, let me reassure you clearly:
Next Steps
✔ 1. Continue Antihistamine Properly
Sometimes cetirizine once daily is not enough.
Doctors may:
• Increase dose (under supervision)
• Shift to levocetirizine / fexofenadine
• Add nighttime antihistamine
Do not self-adjust dose without medical advice.
⸻
✔ 2. Check for Triggers
Ask yourself:
• New food?
• New soap or detergent?
• Outside food?
• Stress?
• Recent viral symptoms?
• New supplement?
⸻
✔ 3. When to Investigate
Testing is NOT required immediately for 3 days of hives.
Investigations are considered if:
• Lasts >6 weeks
• Associated with fever, weight loss
• Swelling of lips/eyes repeatedly
• Breathing difficulty
If thyroid is known, repeat TSH only if symptoms persist.
⸻
✔ 4. Emergency Signs
Go to ER if:
• Lip/tongue swelling
• Breathing difficulty
• Throat tightness
• Dizziness
That suggests angioedema.
Health Tips
✔ Avoid scratching
✔ Avoid hot showers
✔ Wear loose cotton clothes
✔ Avoid alcohol temporarily
✔ Reduce stress
✔ Take antihistamine consistently, not only when rash appears
Most acute urticaria settles within 1–2 weeks.
Since you have thyroid history and ongoing symptoms despite medication, it would be sensible to review your dose and assess whether you need short-term combination therapy.
I recommend booking an online consultation so we can adjust treatment properly and prevent this from becoming chronic urticaria.
Your symptoms are suggestive of acute urticaria (hives), which is a very common allergic skin condition. The appearance of rashes in the evening or night does not mean any serious hidden disease in most cases.
What you can do:
• Continue antihistamine regularly (not only when rash appears)
• Avoid very spicy food, alcohol, and packaged foods temporarily
• Wear loose cotton clothes
• Apply calamine lotion for itching relief
• Maintain good hydration and sleep
Most acute urticaria settles within 1–2 weeks.
When to worry:
• Lip/eyelid swelling
• Breathing difficulty
• Persistent hives beyond 6 weeks
• Fever or joint pain
If symptoms continue, dose adjustment or another medicine may be required.
