Redness and intense itchiness under every toenail, with significant pain on contact (touching shoes, socks, floor, or even light pressure), strongly points to subungual inflammation / paronychia or onychomycosis with secondary bacterial involvement — most likely a mixed fungal + bacterial infection around/under the nail folds. This pattern (all toes affected symmetrically, severe contact pain, redness/itch) is very common in humid climates, after prolonged shoe wear, poor foot hygiene, or minor trauma. It is not dangerous in the short term but is very uncomfortable and can spread or cause nail damage if left untreated.
Next Steps
1. See a dermatologist or general physician within 2–3 days — do not delay, as this needs clinical examination (nail scraping / KOH mount or culture may be done to confirm fungus vs bacteria). • They will likely prescribe: • Topical antifungal + antibiotic combination cream • Oral antifungal if nail plate is involved • Pain relief if needed 2. Do not self-treat with random creams — steroid creams (Betnovate etc.) will worsen fungal infection; plain antifungal alone may not cover secondary bacteria.
Health Tips
• Immediate relief (safe to start today): • Soak feet in warm water + pinch of potassium permanganate (light pink color) or Betadine solution 10–15 min, 2–3 times/day — reduces redness, itch, and bacterial load • After soaking, dry thoroughly (especially under nails) with clean towel/hair dryer on cool setting • Apply plain coconut oil or castor oil thinly under nail edges — soothing & mild antifungal • Wear open sandals or loose cotton socks at home — no closed shoes until better • Pain & itch control: • Paracetamol 500–650 mg every 6 hours if pain is severe • Avoid touching/scratching — keep nails very short & clean • Prevent worsening: • No barefoot walking in wet areas (bathroom, pool) • Change socks daily (cotton only) • Disinfect shoes with antifungal powder/spray (Candid dusting powder inside shoes) • Avoid nail polish/remover for now — traps moisture Most cases improve significantly in 7–14 days with proper topical treatment + hygiene. If it spreads to fingers, pus appears, or severe swelling develops — see doctor same day. For a clear, step-by-step treatment plan (exact creams, oral medicines if needed, how to manage pain/itch at home, and when to recheck), please book an online consultation with me — I’ll review your symptoms in detail and guide you safely so you can get comfortable again quickly. Take care — relief is very close

This does not look like typical nail fungus. Pain and redness under many toenails is more likely skin or nail-fold irritation/infection. Keep feet dry, avoid tight shoes and apply a plain antifungal cream twice daily for 2 weeks. If there is pus or no improvement in 10 days, please see a doctor in person.
