It seems to be fungal infection. Medications: • Itraconazole 200 mg: Twice daily after food, for 2 weeks. • Hydroxyzine (Atarax) 25 mg: After breakfast and lunch, for 10 days (for itching/allergy relief). • Montelukast + Levocetirizine (Montek LC): One tablet at bedtime. Topical Treatment: • Clean the affected area gently using warm water and mild glycerin soap (e.g., Dove). • Apply Cotrimoxazole-based antifungal cream/ointment twice daily after cleaning. Maintain strict control of blood sugar levels. Follow a diabetic-specific diet as advised by a nutritionist or physician.
Next Steps
It is advisable to consult a dermatologist for accurate diagnosis and treatment.

Please consult a skin Specialist in your area for a physical examination. There is extreme thickening of the skin on feet. It could be due to fungal infection.

There is fungal infection.  How much is his sugar and what meds he is taking? Share online. For now he should take tab monteleukast 10mg daily for 7 days. Tab cetrizine 5mg one morning and one evening for 7 days. Tab griseofulvin 500mg one daily for 14 days( not to take alcohol while taking griseofulvin). Apply candid lotion over itchy parts twice daily for 14 days. Then consult online and let me know progress. Accordingly treatment will continue for 6 to 8 weeks further for total clearance.
