You are 38 years old with: • Severe leg and feet pain • Calf cramps • Tightness (more on left leg) • Worse at night • Flare 1 week before periods This pattern strongly suggests hormone-related muscle cramps (PMS-associated) combined with possible: • Magnesium deficiency • Vitamin D deficiency • Iron deficiency • Mild venous insufficiency • Premenstrual fluid retention The fact that it worsens 1 week before periods indicates a hormonal trigger (progesterone drop phase). Night worsening is typical for: • Electrolyte imbalance • Restless leg–type symptoms • Circulation issues If the pain is: • Localized to one calf with swelling/redness → must rule out clot (rare but serious) • Otherwise bilateral cramps → usually metabolic/hormonal Since you mention left leg tightness more prominent, that needs careful assessment to rule out vascular causes if swelling is present.
Next Steps
✔ 1. Important Tests Do: • Serum Vitamin D • Serum Magnesium • Serum CalciumFerritin (iron stores) • TSH • CBC If left leg is visibly swollen: • Doppler ultrasound to rule out DVT ⸻ ✔ 2. Immediate Relief • Warm compress at night • Gentle calf stretching before bed • Magnesium supplement (if not contraindicated) • Hydration • Reduce caffeine • Elevate legs at night ⸻ ✔ 3. Watch for Red Flags Seek urgent care if: • One leg swelling • Sudden severe calf pain • Redness or warmth • Breathlessness These require immediate evaluation.
Health Tips
✔ Start daily calf stretches ✔ Walk regularly ✔ Maintain hydration ✔ Reduce salt before periods ✔ Consider B-complex if dietary deficiency ✔ Track symptoms with menstrual cycle Premenstrual leg cramps are common but severe unilateral tightness should not be ignored. Since your symptoms are recurrent and hormonally patterned, I recommend a structured hormonal and metabolic evaluation through an online consultation so we can rule out deficiencies, assess clot risk properly, and create a targeted treatment plan instead of just temporary relief.

Leg and calf pain with cramps, worse at night and before periods, is most commonly due to magnesium, calcium, or vitamin D deficiency plus hormonal changes (PMS). It is usually not serious. What you can do: • Magnesium supplement at night • Calcium + Vitamin D • Warm compression or massage • Daily calf stretching and good hydration If symptoms continue, check Vitamin D, B12, Calcium, CBC, Thyroid. text me anytime if pain persists or swelling appears.
