Hi doctors I have piles / an anal fissure. If I do push-up exercises, will it cause any problem? Will it increase the pain or burning?

Yes — push-ups can increase abdominal pressure and may worsen pain/burning in piles or fissure, especially if you strain or hold your breath. Light activity is okay, but intense exercises during active symptoms can delay healing.
Next Steps
1. Avoid push-ups/heavy workouts till pain settles. 2. Focus on walking, stretching for now. 3. Treat constipation first (soft stools = faster healing). 4. If pain persists, get examined once so proper treatment can be started early.
Health Tips
Don’t strain during stools or workouts. Drink plenty of water and add fibre. If symptoms aren’t improving, a quick consult helps prevent chronic fissure/piles.

Push-up exercises increase abdominal pressure and straining, which can worsen piles or an anal fissure and may increase pain, burning, and discomfort, especially during the active phase. Hence, it is better to avoid push-ups when symptoms are present.
Next Steps
Prefer low-strain exercises like walking, stretching, or light yoga • Maintain soft stools with a high-fiber diet and adequate fluids • Resume strenuous exercises only after pain and burning have reduced • If symptoms persist, consult a surgeon (me) for proper evaluation
Health Tips
Avoid heavy lifting, straining, and breath-holding during exercise • Constipation and excessive straining can worsen symptoms • Regular bowel habits are key to recovery

Pushup exercise is not recommended as the abdominal pressure /downward pressure can cause existing hemorrhoids to bulge (prolapse) or become more irritated, leading to pain, itching, or bleeding. ‘Not recommended’
Health Tips
If you notice a significant increase in bleeding or a hard, painful lump (which could be a thrombosed hemorrhoid), it’s worth a quick chat with a doctor

Since piles and anal fissure pain is aggravated by breath holding or straining which are not directly caused by pushups, so you can continue with the exercise.
Next Steps
Avoid spicy food and take plenty of roughage diet and fluids.
Health Tips
Avoid heavy exercises and core strength training which involves breath holding.

Push-ups do not directly press on the anus, so they usually do not worsen piles or fissure. However, holding breath or straining during exercise can increase pressure, which may increase pain or burning. Avoid for now Heavy weight lifting Squats with heavy load Exercises that cause strong abdominal pressure
Next Steps
What you should do Do gentle push-ups only (slow pace) Breathe normally — never hold your breath Stop if you feel pain, burning, or bleeding Focus more on walking, light stretching, yoga
Health Tips
Keep stools soft (high-fiber diet, plenty of water) Avoid constipation and straining — this matters more than exercise If pain or bleeding increases, stop exercise and consult a doctor(us).

Usually its not causes any problem. You need to change your lifestyle and dietary habits to stay away from constipation. Constipation make stool hard and its causes anal canal wall rupture that's why fresh bleeding occure. For some days you avoid heavy excercise, after heel  the fissure you may continue your excercise and eat more fibrous diet. Please consult me on practo for further fissure management and medical treatment.

• Push-ups are generally safe in people with piles or an anal fissure because they do not directly strain the anal canal, unlike heavy squats or deadlifts. • However, straining, breath-holding, or increased abdominal pressure can: • Increase anal sphincter tone • Worsen burning, spasm, or pain, especially with an active fissure • If your fissure is painful or bleeding currently, even mild strain can temporarily increase discomfort.
Next Steps
How to do push-ups safely ✔ Breathe continuously — exhale while pushing up ✔ Avoid holding your breath (no Valsalva maneuver) ✔ Keep reps moderate, stop before fatigue-straining ✔ Do them after bowel movement, not before ✔ Stop if you feel: • Anal pain • Burning • Spasm • Pressure sensation Exercises to AVOID for now ❌ Heavy squats / leg press ❌ Deadlifts ❌ Planks held for long durations ❌ Any exercise that makes you strain hard
Health Tips
• Warm water sitz bath after exercise → relaxes anal sphincter • Keep stools soft (fiber + water) • Apply local fissure/piles ointment as prescribed • Avoid workouts if: • Severe pain • Fresh bleeding • Severe constipation Good alternative exercises ✔ Walking ✔ Light cycling ✔ Gentle stretching ✔ Yoga (no breath holding) • Push-ups won’t cause piles or fissure, but poor technique can worsen symptoms • Do them gently, with breathing • If pain increases → pause workouts until fissure settles If you want, tell me: • Do you have pain during stool or bleeding? • Is it fissure, piles, or both? I can guide you more specifically.

Push-ups do not directly increase pressure on the anal area like squats or heavy weightlifting. If done without breath-holding or straining, they are usually safe avoid hold your breath. Avoid straining If fissure/piles are currently very painful or inflamed. Avoid Certain Exercises (especially during flare-up) Heavy weightlifting Squats / deadlifts Intense core workouts (planks, crunches) Cycling for long durations High-impact exercises causing pelvic strain
Next Steps
Most importantly don't make yourself constipated. have plenty of fluids
