As your ECG, TMT and echo results are normal, the symptoms you are experiencing are unlikely related to heart issues, rather anxiety related, neck muscles and nerve related. You can avoid standing while urinating

It seems you have much anxiety. Learn to relax via meditation and deep breathing. Read some good literature in your leisure time. Avoid standing abruptly. For the abnormal sensation, take tab neurobion forte one daily for a month. Get thyroid function tests done and share results.

Based on the details you have shared, your heart-related tests (ECG, ECHO, TMT) are normal and your blood reports are largely within normal limits. This makes a serious heart problem very unlikely. Your symptoms—on-and-off central chest pressure, palpitations, pins-and-needles sensation, cold sweats in early morning, urge to urinate, hot–cold sensations on neck movement, and anxiety on waking—are most consistent with panic/anxiety episodes with autonomic nervous system activation, along with possible cervical muscle/nerve irritation and mild acid reflux. The early-morning symptoms happen due to a natural rise in stress hormones (adrenaline/cortisol), which can cause palpitations, cold sweats, tremulousness, and strange temperature sensations. Neck bending triggering symptoms supports a cervical or muscular cause, not a cardiac one. Your mildly raised SGPT (59) is not dangerous and is commonly seen with stress, fatty liver, or medication use. TSH, sugar, cholesterol, kidney function, and vitamin D are normal.
Next Steps
• Continue Petril-Beta as prescribed; do not stop suddenly • Practice slow deep breathing twice daily • Improve neck posture, avoid prolonged mobile use with bent neck • Avoid caffeine, smoking, alcohol, and late heavy meals • Maintain regular sleep and hydration • Repeat liver function test after 6–8 weeks • If symptoms persist, consult a physician for anxiety management support
Health Tips
• These symptoms are not life-threatening and not due to heart disease • Stress and fear can worsen symptoms, so reassurance is important • Seek urgent care only if there is continuous severe chest pain, fainting, or breathlessness at rest Conclusion: Your condition appears functional and stress-related, and with reassurance, lifestyle correction, and anxiety control, symptoms should improve.
