Internal hemorrhoids do not cause abdominal pain. Your pain is more likely related to a bowel issue like constipation or irritable bowel syndrome(IBS). If symptoms persist, tests like stool examination, ultrasound, or gastroenterology consultation may be needed.

Internal hemorrhoids usually cause painless fresh blood during stools and do not cause abdominal pain. Pain on the left side of the abdomen before passing stools is more suggestive of bowel spasm, constipation, or IBS, not piles.
Next Steps
Continue the piles medicines as advised, keep stools soft, and drink plenty of fluids. If abdominal pain or bleeding continues, you should get further evaluation as advised by your doctor.
