Sir based on the ultrasound breast report showing irregular hypoechoic lesion in right breast with birds -5 lesion it a malignant breast lesion .kindly further follow up with a trucut biopsy .

According to scan attached i would suggest you to visit a breast surgeon asap cuz of the elderly age and plan ur further steps accordingly .
I would also suggest you to get a biopsy done prior or even after visiting your surgeon
