I have been diagnosed with OCD. My symptoms improved after the first review, but during the second review, my psychiatrist prescribed Sritin 50 mg, Unisert 100 mg, Betapill 20 mg, and COCID 40 mg for 4 weeks. Is it safe to take these medications for that duration, and are there any possible side effects I should be aware of? Because after the first review I am facing acid refluxes issue and am afraid that I have gynecomastia. Please advise.

Thank you for sharing your concern. You have been prescribed a combination of medications for OCD and associated symptoms:
Sritin 50 mg (Sertraline, SSRI – first-line for OCD)
Unisert 100 mg (Clomipramine, tricyclic antidepressant - also effective in OCD)
Betapill 20 mg (Propranolol – used for anxiety/tremors, heart protection)
COCID 40 mg (Pantoprazole – for gastric acid protection)
Next Steps
1. Do not stop medication abruptly – that can worsen OCD.
2. Share these side effects with your psychiatrist promptly so dose adjustment or change can be considered.
3. If you notice breast swelling/tenderness, nipple discharge, or persistent reflux, get examined.
4. Maintain lifestyle measures for reflux (avoid heavy meals, caffeine, alcohol, late-night eating).
Your current issues:
a. Acid reflux – despite Pantoprazole, may be worsened by Clomipramine.
b. Fear of gynecomastia – uncommon, but SSRIs and Clomipramine may very rarely contribute via prolactin changes. A physical examination and, if needed, prolactin level can clarify this.
Health Tips
These medicines can be safe if monitored correctly, but your regimen needs close supervision and regular follow-up. Please do not alter treatment on your own.

The medicines prescribed (Sritin, Unisert, Betapill, Cocid) are commonly used in OCD management and usually safe under a psychiatrist's guidance. Side effects can include gastric issues, drowsiness, or hormonal changes in some individuals, which may explain your reflux or concern about gynecomastia.
Next Steps
Do not stop the medicines suddenly. Discuss these side effects with your psychiatrist so they can adjust doses or switch to alternatives if needed. Also, consider a gastro consult for reflux.
Health Tips
Take medicines after meals to reduce acidity, maintain a light diet, and avoid excess caffeine/spicy food. Along with medication, counselling/CBT helps greatly in managing OCD - you may consult me for therapy support.
Do not worry, Sertraline is a good choice in your case, since it is not associated with increased prolactin that can cause gynaecomastia,
*For acid reflux you can continue pantop or other PPI,
Some life style modifications to reduce acid reflux as
*Reduce tea or coffee intake, especially in evening hours
*Try to quit alcohol or smoke if addiction is there
*Try to have a meal before 8 o'clock and dinner should be light
* Just after a meal do not lie down on bed
* Try to opt for a brisk walk, relaxation exercises
"Most important maintain regular follow up with your doctor, maintain records of your symptoms, queries to discuss, trust your doctor"
Gradually Medications will be tapered one by one, as per symptoms response
Do not adjust the dose on your own
Hey,
It’s completely valid to feel uneasy when new medicines are added, especially after already experiencing reflux and body changes. The medicines you listed are usually prescribed for OCD, anxiety, and related symptoms, but they can sometimes cause stomach upset, weight or hormonal changes, or rarely breast tissue swelling in men. What’s important is not to stop or adjust them on your own. Please call your psychiatrist, share clearly about the reflux and your fear of gynecomastia. They can check if one of the medicines is causing it and may adjust the dose or switch. In the meantime, avoid lying down right after meals, reduce spicy or acidic food, and keep track of any changes in your body.. You’re doing the right thing by noticing early and asking.
Next Steps
Be open to your doctor
Health Tips
Don’t fight every thought – let the intrusive thought come, notice it, and then bring your focus back to what you were doing. Fighting makes it louder.
Usually medicines are safe and the doses depends on your symptom profile and the intensity which can be judged only during the consultation and also, your acid reflux might subside gradually but for the time being doses can be adjusted along with an antacid. To check whether you have gynaecomastia or not, again the interview process is necessary to determine and advise the investigation for the same. You can seek a consultation again for second opinion.
