Your iron report does show deficiency — serum iron is 44.9 μg/dL (normal 65-175) and transferrin saturation is 12.77% (normal >20%), which clearly indicates depleted iron stores. Your doctor may have held off because haemoglobin was still normal (pre-anaemic stage), but the deficiency is real and should be treated before it progresses to anaemia. Your B12 of 220 pg/mL is at the very lower end of normal — borderline, and worth monitoring. Increase dietary iron (spinach, lentils, jaggery, meat) and consider iron supplementation after discussing with your physician.
Next Steps
Show this report to your physician and specifically ask for ferrous sulfate or ferrous ascorbate supplementation. Take iron on an empty stomach with vitamin C (lemon water) for better absorption. Recheck iron studies and CBC in 3 months.
