Hello Doctor, my son is 2.5 years old, Monday afternoon woke up with redness on penis top area. He got penis circumcision when he was 1 years old and March 2025 got hydrocele operation done, all was well.
We give him a bath daily and diapers are changed regularly. Yesterday I consulted the pediatrician attached a prescription but no relief. We applied ghee as well. He is crying a lot in pain. Please suggest which doctor we should consult for this condition and why this happened?

Your son might have an urinary infection or sometimes an insect or any bite can also result in this
Make him sit in a warm water bath for 10 min everyday
Wil reduce the pain
Try to avoid diapers temporarily
Next Steps
kindly take him to a nearby pediatric surgeon to be examined. can identify the cause immediately

Please do not apply local applicants like ghee if it's skin erosion clearly visible area.U may not apply aseptic ghee and it will further increase infection risk
It may be superficial inflammation as it's bare area without prepuce
Better go for Oral anti inflammatory drugs
If there is fever and warmth over red area, better go for complete blood picture also
Next Steps
Consult General surgeon and pediatrician
Health Tips
Avoid diapers, avoid moisture , urine or faecal soiling over penis..allow free air over reddened area, Use Local coolents like Calosoft lotion if skin is intact , without erosion
Kindly rule out any injury to that area . It may be an infection glans (Balanitis). Kindly consult in clinic or on video call so that diagnosis can be made .
Next Steps
consult General surgeon
Health Tips
Don't apply Ghee or other such stuff as it may aggravate inflammation.

Possible causes :
1. Balanitis (most likely):
Inflammation/infection of the glans penis, common in circumcised boys.
Causes: bacterial or fungal infection, poor hygiene, irritation from diapers or soaps.
2. Irritant Contact Dermatitis:
From diapers, soaps, creams, or even ghee (which is not sterile and not recommended on wounds or irritated skin).
3. Urinary Tract Infection (UTI):
Can sometimes present with referred penile pain/redness in children.
4. Infection Post-Surgery (unlikely since surgery was months ago, but worth ruling out if signs worsen).
Next Steps
Which Specialist to Consult:
General surgeon – Ideal for this issue, especially with past surgical history.
Pediatric Dermatologist – If skin infection is suspected
Health Tips
Stop applying ghee – Can worsen infection.
Gentle cleaning with warm water only.
Avoid soaps, powders, and antiseptics directly on the penis.
If fever, swelling, pus discharge, or urinary difficulty appear – visit emergency room immediately
Your child has pain and redness in the private part even after using medicine which means the problem needs proper treatment.
It may be due to infection or irritation. Using ghee or home remedies can make it worse.
Please take him to a child surgeon (pediatric surgeon) or child urologist as soon as possible.
Next Steps
Don’t apply anything at home
Keep the area clean and dry
Use only what the doctor gave
Health Tips
Since he is crying a lot, it’s better to get it checked quickly.
