Newborn weight gain challenge
My 21 days old baby has gain only 200gm from his birth weight. He is born in 38 weeks and birth weight is 2.7kg. After 21days follow up checkup, he is 2.9kg. He is on breastfeeding but now doctor suggested me to give 60ml formula in every 3hours. I am not sure if breastmilk is sufficient for him or not. Morning after feed he sleeps peacefully but night he becomes cranky and asks for feed for more than hour. How to increase milk supply in this case?

Hi I think the baby is doing fine. 3 markers for sufficient milk output 1 Daily pee count more than 6-8 times 2 Sleeps peacefully 2-2.5 hours after breastfeeding most of the time 3 Weight gain 25-30 gram per day Important to note that birth weight = day 14 weight due to initql weight loss due to diuresis.

You have to have galactogogues - Shatavari, methi, Gond, etc.. and medication with prescription. I would advise on demand feeding rather than top feeding. Babies tend to cluster feed in the evenings/night. You also should rest well, be stress free for good breast milk output.

At present, the baby is not gaining weight adequately, which is something we need to address early as brain growth is rapid.38 was initial losses also low. The best and first step is to optimise breastfeeding. Effective suckling and complete emptying of the breast are the strongest natural ways to increase milk supply. Many mothers feel their milk is low, but in most cases, supply improves well once feeding technique and frequency are corrected. There are also safe medications and natural foods (galactagogues) that can help improve breast milk production when needed. These work best when combined with good suckling. If formula feeds are started early, breast milk supply can gradually decrease, because the breast is not being emptied enough. Therefore, formula is usually considered only if all breastfeeding optimisation measures fail. With proper guidance and support, most babies start gaining weight well on breast milk alone.

Usually baby loses weight in the first 10 days and then only gains weight. There are a lot of natural and marketed lactogogues available for increasing lactation Also destressing will help

Baby weight gain is 30g/day for 1st 3months starting from 10th day pf life. So ur baby should have gained 300g but has gained 200g. So increase gradually the amount of feeds to 45ml then 60ml. Try to give expressed breast milk once or twice. This way u can measure the amount of feeds u r giving by breastmilk. The more u feed by breastmilk the more will be the production. Mother should be well hydrated

Top feed should not be the ideal way out. If the baby is sleeping peacefully for 2-3 hrs after a feed it suggests that baby is feeding well.Cluster feeding is very common in babies during night,especially between 4-6 weeks of age. You might try adding some natural galactagogues in your diet like saunf,jeera powder ,oats.Meanwhile can consult your paediatrician , who might examine the milk supply and suggest further if anything needs to be done since the baby has slow weight gain

Pls consult your Pediatrician / laction expert Let them check for adequate secretionand examine mothers breast to rule out retracted/ flat nipple and also technique of breast feeding If secretion are less then lactogogues are available
