Hello ! Hello! It’s understandable to feel concerned after losing 8kg in just two weeks post-surgery. Losing 7–8 kg in just 14 days is a significant "red flag" that requires careful investigation. While your Colonoscopy and Fecal Calprotectin results are reassuring, an IBS diagnosis alone usually does not cause such rapid weight loss. We must ensure there are no underlying metabolic issues or post-surgical complications affecting your nutrition.
Next Steps
A)Essential Tests: Please get a Thyroid profile (TSH), Blood Sugar (HbA1c), and Ultrasound of the Abdomen to rule out other causes of weight loss. B)Follow-up: Share these reports here as soon as they are ready so we can adjust your treatment and diet plan. C)Surgical Review: Since these symptoms started a fter your hemorrhoid surgery, a quick follow-up with your operating surgeon is advised to ensure your recovery is on track.
Health Tips
A)Calorie-Dense Eating: To regain weight without stressing your gut, eat 5–6 small meals instead of 3 large ones. Add healthy fats like a teaspoon of ghee or olive oil to your meals. B)Protein: Stick to easy-to-digest proteins like boiled eggs or well-cooked lentils. WORD OF CAUTION : Seek immediate medical help if you experience a high fever, see blood in your stool frequently , or if your abdominal pain becomes severe.

Dear patient Your colonoscopy and reports (very low fecal calprotectin, normal CRP) do not suggest inflammatory bowel disease. Findings are more consistent with IBS, as advised. However, 7–8 kg weight loss in 2 weeks is significant and should be monitored. If weight loss continues, we may need basic tests (CBC, thyroid, sugar, etc.). For now: • Take small, frequent, high-calorie meals • Ensure good hydration • Add soluble fiber (psyllium) if constipation persists • Continue prescribed medicines • Manage stress and maintain proper sleep If you notice blood in stool, persistent vomiting, fever, or continued rapid weight loss, review urgently to surgeon.

You may have post surgical pain, stress, anxiety which leads to reduced gut motility which causes low appetite which leads to reduced intake of food causing loss of weight. Other parameters are normal except mild esr elevation which is non specific..Also there’s no signs of inflammation because fecal calprotectin is low Symptoms you had mentioned are not typical of IBS
Next Steps
-Kindly update if there’s any loss of appetite, family history of cancer,smoking/alcohol habits, occupational exposure of dust or metals, fever -Look for anemia(take cbc test),GI bleed, take thyroid tests, tablet loperamide BD if diarrhea is present -frequently check your weight -tab dicyclomin bd for 3 days which gives anti spasmodic effect -eat proteins of 1-1.5g/day,dry fruits, nuts, green leafy vegetables, avoid caffeine, fatty foods, artificial sweeteners -lower your stress levels by doing meditation, yoga

Please share if you have loss  of appetite?Commonly weight loss occurs after hemorrhoids surgery due to reduced food intake occurring due to fear of painful bowels movements, post operative pain, digestive disturbances, metabolic adjustments during healing. Take a high protein,  high carbohydrate diet,  stay relaxed, stay active, avoid constipation.  Gradually you shall gain weight.

You are 3 weeks post hemorrhoid surgery and now have: • Abdominal pain • Change in bowel habits • Constipation episodes • 7–8 kg weight loss in 14 days • Colonoscopy: few cecal erosions, no active bleeding • Fecal calprotectin
Next Steps
✔ 1. Important: Reassess the Weight Loss You need to confirm: • Was this measured on same scale? • Any fever, night sweats, blood in stool? • Loss of appetite? If weight loss continues → further evaluation required: • Thyroid profile • Serum albumin • Stool occult blood • Celiac screening • CT abdomen (if clinically indicated) Weight loss is the only red flag here. ⸻ ✔ 2. IBS Recovery Plan IBS after surgery is common due to: • Gut-brain axis disturbance • Antibiotics • Stress • Pain anticipation Management includes: • Continue Normaxin short term • Add probiotic (multi-strain) • Start soluble fiber (psyllium husk small dose) • Gradual diet normalization Avoid: • Excess fasting • Severe food restriction • Google-driven fear diets ⸻ ✔ 3. Constipation After Hemorrhoid Surgery Common causes: • Fear of straining • Inadequate fiber • Reduced mobility Use: • Warm water in morning • Stool softener if needed • Adequate hydration ⸻ 3️⃣ How to Increase Weight Safely Your weight gain strategy should be structured: Step 1: Control Gut First (2–3 weeks) Stable bowel habit → weight gain follows. Step 2: Calorie-Dense but Gut-Friendly Foods • Rice + curd • Banana • Boiled potatoes • Peanut butter • Eggs (if tolerated) • Smoothies (banana + peanut + milk) • Small frequent meals (every 3–4 hours) Avoid high-fat heavy meals initially. Step 3: Light Strength Exercise Once pain reduces: • Walking • Gradual resistance training Muscle mass recovery improves weight and confidence.
Health Tips
Your reports: • Do NOT suggest Crohn’s • Do NOT suggest cancer • Do NOT suggest severe inflammatory disease Low fecal calprotectin is highly reassuring. Your biggest issue right now is likely:

Dear Patient, Your reports are reassuring. Normal fecal calprotectin (<5) and CRP indicate no active intestinal inflammation, so serious diseases like IBD are unlikely. After hemorrhoid surgery, bowel habit change, abdominal discomfort, and weight fluctuation can occur due to stress, altered diet, gut sensitivity, and IBS-D. Rapid weight loss over 2 weeks is often due to reduced intake, anxiety, and fluid loss rather than true tissue loss. What you should do to gain weight and improve IBS: • Eat small frequent meals (5–6 times/day) instead of large meals • Add calorie-dense but gut-friendly foods: banana, rice, curd, oats, boiled potatoes, peanut butter, ghee in small quantity • Include protein: eggs, paneer, dal, chicken, whey isolate (if tolerated) • Avoid triggers: very spicy food, excess tea/coffee, fried food, artificial sweeteners • Continue Normaxin as prescribed (helps gut motility and pain) • Probiotic daily for 3–4 weeks (like VSL#3 / Darolac) • Manage stress — IBS strongly links with anxiety (yoga, breathing, walking) • Light exercise improves appetite and bowel rhythm Weight usually improves once gut symptoms settle. Monitor weight weekly, not daily. Consult me if persistent severe pain, blood in stool, fever, or continued weight loss occurs. For further guidance and treatment, text me on Practo anytime.
