Vitamin B12 normal range is between 200 to 900 nanograms/ ML  Since your level is 183 it lower than normal    Vitamin D level  less than 10 ng/ml is severe deficiency 10 to 24,ng/ml is Mild to moderate deficiency .As per report you do have VitB12 and Vit D deficiency.1)  To increase level of vit B12 if you are non vegetarian Fish and shell fish chiken, eggs are good source in addition Fortified breakfast cereal ,low fat milk yogurt and cheese are good source of vit B12  As a treatment you can take oral vit B12: in the form of methycobalamine 500 to 1000mcgbcan be taken for atleast 3 months

The report shows decreased level of both vit d and vit b12. You should start consuming b12 rich food as well as b complex supplement . For vit d best option is to sinbath daily 15 to 20 min and hopefully your muscular pain will be alleviated.
