Loose motions for 4 months baby

Asked for Male, 1 Years

My baby 4 months old. Suffering with loose motions like water. Doctor suggested Sporlac sachet. Can you please suggest how to use. Do we use with water or mother feed. Till now we haven't used water for baby. Can you suggest the better one asap.
