Do the following,
1. Please show your medications to a doctor to make sure they have nothing to with your abnormal reports and ask them to measure your blood pressure initially when you are lying down and then after you stand up to check for a drop.
2. If you are a smoker and or losing weight- Get a CXR done
3. Blood tests: Vitamin D, Cortisol, TSH, HbA1C and Osmolality
4. Urine test: Sodium and Osmolality
5. Medications:
i) Calcium tablets
ii) Methacobalamin tablets once a day (long term)

✅ Vitamin B12
If symptomatic (fatigue, tingling, hair fall):
Best option:
• Inj. Methylcobalamin 1000 mcg IM weekly × 4 weeks
Then
• Monthly × 3 months
OR
• Oral methylcobalamin 1500 mcg daily for 3 months (if no absorption issue)
⸻
✅ Vitamin D
• Cholecalciferol 60,000 IU once weekly × 8 weeks
Then
• Once monthly for maintenance
Take with fatty meal.
⸻
✅ Calcium
• Calcium citrate 500 mg daily (along with Vitamin D)
⸻
✅ Sodium 128
Important:
Repeat sodium in 3–5 days.
Meanwhile:
• Do NOT drink excessive water
• Maintain moderate salt intake
• Check if any vomiting/loose motions/thyroid issues
If sodium stays
Health Tips
For B12:
• Eggs, milk, paneer, curd
• If vegetarian → supplementation required long-term
For Vitamin D:
• 20 minutes sunlight daily (11 am–2 pm)
• Weight-bearing exercise
For Sodium:
• Avoid overhydration
• Check thyroid (if not done recently)
⸻
Summary
Most important issues:
1. Vitamin D deficiency (severe)
2. Vitamin B12 deficiency
3. Low sodium (needs repeat check)
These are correctable conditions.
⸻
If you tell me:
• Your age
• Any symptoms like tingling, weakness?
• Are you vegetarian?
• Any thyroid disorder?
I can fine-tune dosage.
Since sodium is 128, I would strongly advise repeating electrolyte levels soon — better to evaluate early rather than delay.
Take tab cholecalciferol 50,000 units once weekly for 6 weeks.
Tab neurobion forte one daily for 6 weeks.
Please share details of any problem you have, any medicine you are on, as sodium is also low. Then I can help you further.
Vitd3
Uprise d60k once a week for eight weeks followed by one per month for six months a complete therapy by WHO. After breakfast.
Vit b12
Neurokind od one after dinner for two months.
KFT is overall normal, uric acid is slightly high ,so you need to increased plenty of water. B12 and Vitamin D level is too low , you need to start proper treatment and medicine for this.consult me on practo for further management and evaluation.
