TSH values are physiologically higher in the first days of life and gradually fall over 2–4 weeks. A TSH around 19 at day 12 is elevated enough to require follow-up, but by itself it does not automatically confirm congenital hypothyroidism unless Free T4 is low or repeat TSH remains elevated/rises. That’s why the important next step is: • Free T4 • Repeat TSH in follow-up • Correlation with newborn screening report Persistent TSH >10–20 beyond 2 weeks, especially with abnormal FT4, is taken more seriously and may need treatment for the same.

Only TSH Value is available. It is borderline value. Please repeat the test after 1-2 weeks TSH and FT4. The normal value for TSH 10 around 3-4 weeks of age. Child requires ultrasound examination and some more tests based on the values.
Next Steps
Child requires clinical evaluation and recommendations. Mother also requires investigation.
