What's her age? Widal is not confirmatory. To diagnose typhoid in blood test is challenge. Always there should be a clinical correlation. Typhoid patients have high grade continues fever. Unless otherwise we treat them, they will become more sick. First thing is check her temperature if it is above 99 then you can go for fever profile test. Does she have any other co morbidities like diabetes
Next Steps
do routine blood tests CBC, FBS, PPBS, RFT, THYROID PROFILE.
Health Tips
also find out whether she has psychological stress, sleep disturbance and snoring
Please update with detailed report and symptoms. If it is widal it’s not confirmatory as it gives many false positive results. As your mother becomes aged she needs to take some additional investigations to confirm whether there is any GI,cardiac, renal issues
Next Steps
Maintain adequate food hygiene
Avoid outside foods, excess carbohydrates, fried and spicy foods
Drink adequate water

If your mother’s test is showing “typhoid positive 60%”, it is most likely referring to a Widal Test result, which can sometimes remain positive for a long time even after past infections. Because of this, Widal test alone cannot reliably confirm active Typhoid Fever, especially if similar results appear every year.
If she gets fatigue around the same months each year, it may not always be recurrent typhoid. Other causes such as seasonal infections, nutritional deficiencies, or chronic infection carriers can produce similar symptoms. Therefore, relying only on Widal results may lead to confusion.
Next Steps
• If symptoms suggest typhoid (fever, weakness, abdominal discomfort), the more reliable test is Blood Culture Test, which confirms active infection.
• During confirmed typhoid, doctors usually treat with antibiotics such as Azithromycin or Cefixime, depending on clinical evaluation.
• If recurrent infection is suspected, the doctor may also check for a carrier state or evaluate hygiene and water sources.
• Blood tests like CBC and liver function tests may help assess fatigue and recovery.
Health Tips
• Drink safe filtered or boiled water, as contaminated water is a common source of typhoid infection.
• Maintain strict food hygiene, especially avoiding outside food during summer months.
• Wash hands properly before eating and after using the toilet.
• Ensure she maintains good nutrition and hydration during recovery.
Since the symptoms seem to occur every year, it would be helpful to review her complete reports and symptoms in detail to confirm whether it is truly recurrent typhoid or something else. You may consider booking an online consultation so the reports can be evaluated properly and the right long-term approach can be planned.
Widal test alone is not reliable for diagnosing typhoid and can remain positive from past infection.
Advice:
Confirm with blood culture or Typhi IgM test during fever
Maintain good food and water hygiene
Consider typhoid vaccination if recurrent risk.
