It's most probably anal fissure which happens to be superficial tearing of your anal mucosal layer due to constipation or dietary and gut issues. It's unlikely hemorrhoids as no tissue or skin tag is visible.
Next Steps
Do visit a surgeon for examination and confirmation.
Health Tips
Until then do sitz bath two times a day for any sepsis prevention and control and prompt recovery with a healthy rich in fibre diet.

It’s either a anal fissure or hemorrhoids Likely cause is low fibre diet, straining on the toilet ,hereditary etc A sitz bath helps in either situation along with stool softeners It’s better to get it checked out just to be safe
Next Steps
First step would be to visit a general surgeon at a nearby clinic
Health Tips
Do not strain on the toilet , drink lots of water and have lots of fibre in diet

Hey sorry to hear that you are in discomfort. Most commonly either hemorrhoids(internal)/piles or anal fissures are most common possibilities. However, an anal examination is very much necessary to evaluate further. Keeping bowel clean and empty, i.e. avoiding constipation or heavy outside food is a starting point.
Next Steps
sitz bath helps to ease comfort. green leafy veggies.
Health Tips
in case the bleed continues, or worsens, best is to visit or consult a physician. take care.

Please be a little considerate towards your body. With these kinds of symptoms and your age being 18 years you should have consulted a General Surgeon in your area much earlier. Anyway, it's never too late. Please consult a General Surgeon in your area for a physical examination.

What you are describing most commonly fits an anal fissure. Typical features matching your symptoms: • Pain and discomfort during and after passing stool • Fresh bleeding only while using the toilet • Burning or sharp pain at the anus • Symptoms present for a few days An anal fissure is a small tear in the anal lining, usually caused by: • Hard stools or constipation • Straining during bowel movements • Repeated wiping or aggressive cleaning Less commonly, it could be inflamed hemorrhoids (piles), but fissures usually cause more pain, whereas piles often bleed with little pain. This is very common and usually not serious, especially when symptoms are recent.
Next Steps
1. Relieve stool hardness (most important) • Start a stool softener (like isabgol or lactulose) • Ensure daily smooth bowel movements 2. Local care • Warm sitz bath (sit in warm water for 10–15 minutes, 2–3 times/day) • Apply a local fissure/pain-relief ointment (as advised by a doctor) 3. Pain control • Avoid straining • Do not delay going to the toilet 4. Consult a doctor (General Physician or Surgeon) if: • Pain or bleeding persists beyond 1–2 weeks • Bleeding increases • Severe pain continues despite treatment
Health Tips
• Drink plenty of water (2.5–3 liters/day) • Eat high-fiber foods (fruits, vegetables, whole grains) • Avoid spicy food and alcohol temporarily • Do not over-clean the area; gentle washing is enough • Avoid sitting on the toilet for long periods In most cases, this condition is benign, treatable, and reversible with simple measures. Early care prevents it from becoming chronic. You’re doing the right thing by addressing it now—help is straightforward and relief is very achievable.

I know this is worrying — from your symptoms (pain + fresh bleeding during toilet) and the photo, this looks most like an anal fissure or irritated hemorrhoid, usually triggered by hard stools or straining. It’s uncomfortable but common and usually not serious if treated early.
Next Steps
• Keep stools soft: plenty of water + fibre • Warm sitz baths 10–15 min, 2–3× daily • Apply a local fissure/hemorrhoid ointment (after cleaning) • Avoid straining and long toilet sitting If pain or bleeding continues beyond a few days, please take a consultation so I can guide proper medicines and rule out anything else.
Health Tips
Avoid spicy food for now. Don’t ignore ongoing bleeding. If pain worsens, swelling increases, or bleeding becomes frequent — consult urgently.
