Cirrhosis, uncontrolled bp on carvedilol
Mother is currently on carvedilol 3.125 mg 1 twice for high bp n compensated asymptomatic liver cirrhosis.  BP remains in 150 sys sometimes fluctuates  n remains130, 145. Rarely reached 160 up. Diastolic 70s-80s to at times 90 up. HR 70-80.  Occasionally even after bp med, bp remains in 145 up.  Gets heavy headed when stressed or very high bp. 71 yrs female. Mild pedal edema. Prediabetes. Subclinical hypothyroidism. Taking Istamet, Thyroxine, multivitamin. Cardiologist suggested stop carvedilol and start atenolol selective betablocker 50 mg morning n 25 mg night. Liver doctor said continue carvedilol and start tenormin as suggested. *Can there be any issues initiating this ? *What alternative advice apart from this is suggested ?

Hi.. Patients with compensated cirrhosis with portal hypertension should be on Non selective beta blockers like Propranolol and Carvedilol. In this case, as BP is on higher side, it would be prudent the to increase CARVEDILOL to 6.25mg twice daily and monitor BP. ATENOLOL does not decrease portal pressures in patients with cirrhosis.  Hence, no additional benefit, liver point of view.
