Left shoulder and neck pain after waking up is often due to muscle spasm or a cervical nerve issue from sleeping in an awkward position. Along with painkillers, try applying a warm compress on the neck and shoulder to relax the muscles. Gentle neck stretches and avoiding a pillow that is too soft or too high can also help. If pain continues beyond a week or worsens, get a cervical spine X-ray done.
