PCOS related weight gain is common even with less eating due to hormonal imbalance. It’s manageable with the right plan. I can guide you step by step feel free to consult.
Weight gain is a symptom of underlying disease and in your case it seems to be PCOS and obvious stress leading to high cortisol in your body which leads to rounded farce and weight gain all over the body.
There are medications for PCOS based on your hormones/ fasting insulin which do help in weight reduction too. Please consult a Gynaecologist for this.
Since you are taking a lot of stress for this issue, you need to take care of your stressors too.
Consciously remove them from your life. Do left nostril breathing 10 times whenever you expect a stressor. Take to some meditation technique which shall give you a constant relaxed and courageous mind and heart to fight the stresses of life like the Heartfulness meditation which can be done through Heartfulness app with an online trainer who does not know you and helps to meditate with you.

Extra stress needs to be avoided even if you dieting or exercises to fight your weight.
Hello. I have reviewed your concerns regarding your weight gain and facial changes.
It is understandable that these changes are distressing, especially when you feel you aren't eating much. In patients with PCOS, weight management can be very challenging due to hormonal imbalances and insulin resistance. However, your specific symptoms require a detailed evaluation.
Potential Causes to Investigate
Insulin Resistance: This is very common in PCOS and makes the body store fat more easily, even with a low calorie intake.
Hormonal Imbalance: Beyond the thyroid, other hormones can influence fat distribution and energy levels.
Cardiovascular Fitness: Your feelings of breathlessness and tiredness during exercise should be checked to ensure your heart and lungs are responding well to physical activity.
Recommended Next Steps
Laboratory Tests: In addition to your thyroid, I recommend checking your HbA1c (blood sugar), fasting insulin levels, and a lipid profile.
Physical Examination: A doctor should physically assess the "roundness" of your face and check your blood pressure.
Specialist Consultation: I strongly suggest consulting an Endocrinologist. They specialize in hormonal issues like PCOS and can provide a more targeted treatment plan to help manage your weight and energy levels.
Lifestyle Management
Dietary Focus: Since you have PCOS, focus on a low-glycemic index (Low-GI) diet. This helps manage insulin levels. Focus on whole grains, legumes, and plenty of vegetables.
Activity: Since you experience pain and breathlessness, do not push yourself too hard. Try low-impact activities like slow walking for 10–15 minutes a day and gradually increase as your stamina improves.

"In a 34-year-old female with a known history of PCOS/PCOD, rapid weight gain accompanied by a 'round face,' breathlessness, and extreme tiredness is a clinical red flag. While PCOS naturally causes insulin resistance and weight gain, we must also rule out Cushing's Syndrome (excess cortisol) or significant anemia, as the 'round face' and breathlessness suggest a systemic hormonal or cardiovascular strain beyond simple weight gain
Next Steps
the tiredness and 'round face' are your body's way of saying the metabolic balance is off. Please book a detailed consultation so we can investigate these symptoms beyond PCOS and get you on a path to feeling light and energetic again."
