Your reports show: • Vitamin D: 13 ng/mL (deficient) • Vitamin B12: 125 pg/mL (deficient) • Homocysteine: 56 µmol/L (elevated, likely secondary to B12 deficiency) • LDL: 148 mg/dL (borderline-high) You’ll need supplementation and some dietary adjustments. ⸻ Plan: 1. Vitamin D deficiency: • Tab/Sachet Cholecalciferol 60,000 IU once weekly for 6–8 weeks, then once monthly as maintenance. • Take after a meal containing fat (for better absorption). • Include sunlight exposure (15–20 min/day). 2. Vitamin B12 deficiency: • Injections: Mecobalamin 1000 mcg IM once weekly for 4 weeks, then once monthly or • Oral: Tab Methylcobalamin 1500 mcg daily for 2–3 months. • Alternate option: Tab Neurobion Forte once daily for 2–3 months. 3. Elevated Homocysteine: • Will improve once B12 is corrected. • Consider adding Folic acid 5 mg daily for 1–2 months. 4. LDL 148 mg/dL: • Adopt a low-cholesterol diet (reduce fried, processed foods, red meat, and high-fat dairy). • Regular exercise, at least 150 min/week. • Recheck lipid profile in 3 months. ⸻ Follow-up: Repeat Vitamin D, B12, and lipid profile after 3 months.

Hello, I am Dr. Romain Rajan, General Physician. Your case has been allotted to me. I understand your concern. Your reports show Vitamin D deficiency (13 ng/ml), Vitamin B12 deficiency (125 pg/ml), and high homocysteine (56 µmol/L) — all of which are interlinked. Elevated homocysteine occurs due to low B12 and sometimes low folate, which can increase long-term cardiovascular risk. Your LDL of 148 is also mildly elevated. These findings are very common even in active individuals and can be corrected effectively with supplements and minor dietary changes. 1. Advices Get 20–30 minutes of sunlight exposure on arms and legs daily. Include more protein, leafy vegetables, eggs, and dairy products in diet. Avoid excessive alcohol, processed food, and sugary snacks. 2. What to Monitor Fatigue, numbness, or tingling in hands and feet. Any muscle cramps or mood changes. 3. Management Please start the following: Vitamin D3 (60,000 IU) – one capsule weekly after meals for 8 weeks, then once every 15 days as maintenance. Methylcobalamin (B12) – 1500 mcg tablet once daily after breakfast for 3 months. Folic Acid (5 mg) – once daily for 3 months (helps normalize homocysteine). Omega-3 supplement (Fish oil 1000 mg) – one capsule daily after meals for 3 months (helps reduce LDL and inflammation). After 3 months, repeat Vitamin D, B12, Homocysteine, and Lipid Profile to assess progress. If LDL remains high, a detailed diet plan or mild statin may be considered. 4. When to Consult a Doctor Immediately If you develop persistent tingling, fatigue, or dizziness. If you start new medications or have thyroid or liver issues. Every patient’s situation is unique. With proper supplementation and diet, your values should normalize within 2–3 months, and homocysteine will drop significantly once B12 and folate improve. 👉 You can reach out to me directly for this case, and also connect with me in future for any medical consultation. I am available on Practo or on WhatsApp at 85271646 seven seven for a one-on-one consultation, where you’ll receive: • A personalized supplement and diet plan • Safe medicine adjustments (if required) • Quick follow-ups to track your recovery Take care, wishing you good health. Best regards, Dr. Romain Rajan General Physician
