Urine culture clear, pain in private part
I have been having pain after urinating for 2 weeks. I took antibiotics for 1 week and my pain didn't subside. So last week I took a urine routine and culture test. Reports are negative. But my pain hasn't subsided. When I have an urge to urinate, the pain is more. After urinating, for sometime there is no pain, but then again I experience burning sensation/uncomfortable feeling throughout the day. I am  having fungal infection in my private part since 2 years. It is chronic, not going away. I am worried what could be the cause for this pain. Kindly advise

The symptoms described, such as pain after urination, burning sensation, and chronic fungal infection in the private area, could be due to multiple underlying conditions even though the urine culture is clear. Here are some potential possibilities: 1. Urethritis or Chronic Prostatitis: Inflammation of the urethra or the prostate can cause similar symptoms. It may not always show up in a urine culture test, and additional tests (like semen culture or prostate fluid analysis) may be needed.    2. Interstitial Cystitis (IC): This is a condition where the bladder becomes painful, causing discomfort during urination, even without infection. 3. Fungal Infection: Since the patient has mentioned a chronic fungal infection for two years, there’s a possibility of persistent irritation. Fungal infections in the genital region can cause long-term discomfort, especially if untreated or improperly treated. A consultation with a dermatologist or urologist for better antifungal management might help. 4. Neurological Factors: Sometimes, pain after urination may be linked to nerve issues in the pelvic region. 5. **Consider Imaging**: A **USG of the whole abdomen** or a **pelvic ultrasound** can provide insights into any structural abnormalities that may be causing the symptoms. **Treatment Approach**: - Stay well-hydrated. - Avoid irritants like caffeine and alcohol which can worsen bladder pain. - Follow up with antifungal treatment for the ongoing fungal infection. - Pain relief with antispasmodic medications like Zerodol-SP can provide symptomatic relief as recommended. Advice It is crucial to consult with me for a detailed examination. I may recommend additional diagnostic tests like cystoscopy or uroflowmetry to rule out less common conditions.

Pain is not due to fungal infection and these to things have to be treated seperately. For fungal infection proper antifungal course has to be taken. For pain during urination detailed workup is necessary .

Burning sensation after urination could be due to a number of possible reasons, including: Urinary tract infection (UTI) A common cause of a burning sensation after urination, UTIs are usually caused by bacteria from the body. Other symptoms include frequent urination and changes in urine color. UTIs are typically treated with antibiotics. Kidney stones Hard masses of minerals and salts that can form in the kidneys and move through the urinary tract. If a kidney stone reaches the tip of the urethra during urination, it can cause a burning pain. Other conditions Other possible causes include painful bladder syndrome, urethral stricture, prostatitis, and kidney disease. Treatment for a burning sensation after urination depends on the underlying cause. For example, UTIs and STIs are often treated with antibiotics. Drinking plenty of water and avoiding irritants like caffeine or spicy foods can also help.
