Tight sphinchtor
A few months ago I had anal fissure and after a lot of medications and efforts, 2 weeks ago Dr. Said now it is healed. I also feel no scratch or tear while applying ointment but from last 3 days my anus has become so much tight that even I can't apply ointment easily and cannot defecate my bowels easily, though there is no tear and scratch in the area. I also feel pressure in anus. What should I do now? I'm doing sitz bath 2 times daily and GTN+ Xylocaine 2 times daily but anus is still very tight.

Physical examination is required to assess the peri-anal muscles tone and then decision can be done regarding the management. If you are having no relief with Sitz bath and GTN ointment, then you have to undergo surgical management.
