It sounds like you’re having a recurrence of symptoms after laser fissure surgery, which can sometimes happen if the internal sphincter muscle remains tight or if healing was incomplete. Even when motions are soft, a small area of spasm or fragile scar tissue can reopen, causing pain and fresh bleeding. This doesn’t always mean the surgery has failed — but it does mean you may need medical re-evaluation to check for residual fissure, infection, or internal hemorrhoid that might be contributing.
Next Steps
• Consult your colorectal or general surgeon again for a proctoscopy to confirm the cause. • For now, follow these steps: • Warm Sitz baths (sit in warm water for 10–15 minutes, twice daily). • Apply topical diltiazem or nifedipine ointment (as previously prescribed) 2–3 times daily to relax the sphincter and promote healing. • Use a stool softener (lactulose syrup or Isabgol) to keep stools soft. • Avoid spicy foods and prolonged sitting. If pain or bleeding persists beyond 1–2 weeks despite these measures, you may need a follow-up laser touch-up or Botox injection to relax the muscle further.
Health Tips
Anal fissures can take time to heal completely — even after surgery. Consistency in local care, diet, and Sitz baths is key. Most recurrences heal again with conservative management, but early review by your surgeon ensures it doesn’t progress to a chronic non-healing fissure.
