Thoracic lymph nodes tuberculosis to
I've had a fever since last month. As long as the medicine is being taken, there is no fever, and as soon as the medicine is over, the fever starts coming again. I have no other symptoms except fever — no weight loss, no cold, or cough. My X-ray is clear. That’s why the doctor suggested a CT scan, CT scan report: MULTIPLE ENLARGED PERIPHERALLY ENHANCING LYMPH NODES NOTED IN MEDIASTINAL, RIGHT PARATRACHEAL, PRECARINAL, SUBCARINAL AND RIGHT PERIHILAR REGIONS — MEDIASTINAL & RIGHT HILAR LYMPHADENOPATHY, KINDLY TO RULE OUT POSSIBILITY OF KOCH'S. Then the Mantoux test, Induration was 22 mm. The doctor gave me medicine — tablets of HRZE fixed-dose combination, each one contains : [Rifampicin 150mg + Isoniazid 75mg + Pyrazinamide 400mg + Ethambutol 275mg] — and told me to take 4 tablets at once, at any time in a day. Could you please confirm whether this is active TB or latent TB, and if the treatment given is correct for my condition?

Findings suggest tuberculosis lymph node TB. Starting HRZE treatment is correct. Fever coming back when you stop medicine means treatment should be taken regularly without breaks. This condition improves well with proper course, but it needs monitoring dose, duration, liver function and response to therapy. I’d suggest book a consultation follow up to ensure everything is going in the right direction.

.Based on your clinical history of persistent fever, the CT scan report showing multiple enlarged, peripherally enhancing mediastinal lymph nodes, and a highly significant Mantoux test (22mm induration), the diagnosis of Thoracic Lymph Node Tuberculosis (Extra-pulmonary TB) is clinically confirmed. Since you are symptomatic (fever), this is considered Active Tuberculosis, and the HRZE fixed-dose combination treatment started by your doctor is the standard and correct protocol for this condition
