Social anxiety and depression.
When I come out feel extreme self conscious and people conscious, feel panicked, extreme unnecessary urge to cry, intense sad and dull feeling inside, difficulty concentrating when talking to people, or having eye contact, get easily hurt and offended, always cautious of not getting embarrassed.

Hi. From what you have mentioned, it seems that you are suffering from symptoms of underlying social anxiety disorder. It manifests as getting anxious in social situations with the fear of doing something embarrassing which in turn leads to avoiding social situations. You should get yourself evaluated completely by a psychiatrist. Treatment with medications and cognitive behavioural therapy will help you recover.
Next Steps
Seek a psychiatric consultation.
