Blood test values will depend on the normal reference range provided by the lab where the test was done. LH, FSH and testosterone values should be compared to normal range for 6 month old baby boys. FSH and LH appear slightly elevated. There is a process called "minipuberty" which starts in the first week of life which can produce high FSH and LH which can last till 3-6 months of life in both baby boys and girls. This causes increased sex hormones which allows for development of genital organs. Around 6 months of age, this "minipuberty" switches off and remains dormant till puberty. So, generally speaking, after 6 months, levels of FSH and LH come down to pre-pubertal range. Premature babies and small for gestational age babies may also have higher FSH and LH levels.
Next Steps
See a paediatric endocrinologist who will take a full history of prematurity, birth weight, neonatal thyroid screen, whether there were any problems with low blood glucose after birth. The doctor will also assess the weight, length of the baby and also the size of the penis and testes. The doctor will also assess whether both testes have descended into the scrotum. The paediatric endocrinologist will also need to perform repeat blood tests after the clinical assessment.
Health Tips
The penis may sometimes appear small because of fat over the pubic bone. The endocrinologist will assess the correct length of the penis.
