Do the following if have not already done,
1. Blood pressure check
2. Urine Microscopy and Albumin-Creatinine ratio
3. Ultrasound kidneys and urinary bladder
4. Blood tests- HbA1C, CBP, ESR, Calcium, Phosphate and Bicarbonate levels

S. Creatinine is an important marker of your kidney. It helps to analyse how your kidneys are doing. Elevated Creatinine along with reduced eGfr would indicate underlying kidney disease. Sometimes it also relates with untreated Hypertension or Uncontrolled Diabetes mellitus.
So, monitoring is required once in two months atleast.
Next Steps
Stay hydrated. check your BP and sugar levels just to make sure they're not a reason for the raised creatinine.
Recheck your Creatinine along with eGFR after a month. If it remains the same, then you need a proper medical attention, to bring it back to normal range.
