From the provided X-ray, there is no obvious fracture, dislocation, or serious structural abnormality visible in the cervical–upper thoracic spine upper back and neck pain for one month with a near-normal X-ray is most commonly due to muscle spasm, poor posture, or cervical strain, especially with prolonged sitting or phone/computer use X-rays do not show muscles, discs, or nerves well, so pain can still be present despite a normal report if pain persists increases, or is associated with arm pain numbness, or tingling an MRI cervical spine may be required.
