Piles can be managed conservatively depending on grade of disease.
But if your grade of disease is more you need to get operated.
A surgeon needs to do examination and decide what needs to be done
Next Steps
Consult surgeon for need of conservative /surgical management.
Health Tips
Avoid straining and sitting in bathroom for longer duration.

From the description, it sounds like prolapsing pile mass (grade 3/4). Where it is internal prolapsing or just external pile mass, a thorough general surgery consultation is needed for examination..
Next Steps
do sitz bath with luke warm water for 5 to 7 days twice daily..
avoid constipation, take laxatives if needed.
apply any cream (smuth, dobesoft) locally thrice daily x 7 days .
