Palpitations can be due to anxiety, stress, caffeine, dehydration, thyroid issues, anemia, electrolyte imbalance, or sometimes medication side effects. Escitalopram is generally safe, it can sometimes cause palpitations, especially if it affects the heart rhythm. Holter monitor (24–48 hrs) or event monitor to catch abnormal rhythms when they happen. Blood work up: thyroid function, electrolytes, hemoglobin. Review caffeine, alcohol, stimulant, or nicotine use. Assess stress/anxiety, since they can mimic or worsen palpitations
Next Steps
Blood workup and Holter monitoring is suggested

What work are you in ? Lots of late night jobs can cause cortisol levels to raise in morning which may cause your problem Anxiety or Panic Attacks: This is a very common cause of palpitations, especially in someone taking an antidepressant like escitalopram. Anxiety can also lead to physical symptoms like bloating and a feeling of being on edge. Gastrointestinal Issues: Conditions such as acid reflux (GERD), gastritis, or irritable bowel syndrome (IBS) can cause bloating and sometimes trigger palpitations Have you ever been evaluated for hypothyroidism? If not kindly evaluate for that

You’re having palpitations quite often in the past few days. Since your blood pressure, oxygen, and sugars are fine, it doesn’t look urgent, but the best next step is to get a check with a cardiologist. They can do an ECG or a Holter test, which records your heartbeat over 24 hours, to see if the rhythm is normal. since you’re on the heavier side, check with a house inmate whether you snores during sleep if yes get in contact with me for evaluation.

Tab. Pantoprazole 40 mg OD (empty stomach) × 10 days • Tab. Simethicone 80 mg TDS (after meals) × 5 days • Avoid excess caffeine, alcohol, smoking • Small frequent meals; avoid late-night heavy meals, carbonated drinks • Stress reduction: breathing exercises / meditation • Continue Escitalopram as prescribed by psychiatrist • Investigations: ECG (repeat), Serum electrolytes, Thyroid profile • Review with physician if palpitations persist >1 week, worsen, or associated with chest pain, syncope, severe breathlessness ⸻ Palpitations may be linked to gas/bloating, anxiety, or rarely drug side effects. Since ECG was normal recently and there are no red flags, supportive treatment + monitoring is usually sufficient.
