A painful breast lump in a 61-year-old person warrants prompt evaluation and should not be ignored. What should be done next? 1. Clinical examination by a General Surgeon or Breast Surgeon as soon as possible. 2. Bilateral mammography (both breasts). 3. Breast ultrasound (USG) to assess whether the lump is solid or cystic. 4. Depending on imaging findings, a FNAC (Fine Needle Aspiration Cytology) or preferably a core needle biopsy may be required to establish a diagnosis. Possible causes A painful lump can be due to: * Breast cyst * Breast abscess/infection * Fat necrosis * Benign breast tumors * Breast cancer (pain does not rule out cancer) Seek urgent consultation if there is: * Rapid increase in size * Skin dimpling or ulceration * Nipple retraction * Bloody nipple discharge * Redness, warmth, or fever * Enlarged lymph nodes in the armpit My recommendation Given the age (61 years) and the presence of a persistent breast lump that has recently become painful, I would advise: * Do not wait or treat empirically. * Arrange a breast imaging workup (mammography + ultrasound) within the next few days. * Follow this with biopsy if imaging is suspicious. The majority of breast lumps are not cancerous, but at this age every new breast lump should be investigated thoroughly before reassurance can be given.

You need to go for a HRUSG of swelling breast plus guided FNAC to know the cause and diagnosis.
Next Steps
If excessively painful with pus discharge then you need to visit a general surgeon immediately as it could be an abscess.
