Hi Doctor, i discussed my foreskin not covering glans from many years ago issue with a urologist here on practo as i live in a village so we don't have any such specialist there, he just told me like you have a short foreskin that's why your glans is not covered. But as mild inflammation is visible under my penis foreskin which causing my inner tip of outermost foreskin tip to fuse with the shaft, i think can due to this reason my penis foreskin not able to retract back to cover glans. I asked this with dr he told me this is normal. Should i check this issue with a dermatologist if it's due to any fungal infection? Also urologist prescribed pregeb forte tablet for 1 month as i am facing discomfort with exposed glans causing pin and needle type sensation when rubbing with clothes. Really if this issue is due to short foreskin how should i move ahead in my life and make my mind this as normal as rubbing of glans distracting my mind from my work.

From the photos, the glans being partially exposed can be due to naturally short foreskin/retracted foreskin, and it does not look like an emergency. But there does appear to be mild inflammation/irritation (balanitis) with dryness, which can cause burning, hypersensitivity and discomfort from rubbing against clothes. Yes, consulting a dermatologist is reasonable to rule out fungal infection, dermatitis or chronic inflammatory conditions. Keep the area clean and dry, avoid strong soaps/antiseptics, use loose cotton underwear and avoid repeated pulling/retraction. Sensitivity of exposed glans usually improves gradually over time as the skin adapts. If redness, white patches, cracking, discharge, severe pain or tightening increases, then in-person urology review is important because sometimes topical treatment or minor procedure may be needed.

The discomfort you are feeling—specifically the 'pin and needle' sensation when your glans rubs against clothing—is a common issue for men with a naturally short foreskin or those who have undergone circumcision. This is known as Glans Hypersensitivity. Because the glans is normally protected by the foreskin, constant exposure makes the nerve endings over-reactive to touch. The 'mild inflammation' or 'fusing' you mentioned needs a closer look to rule out Balanitis or minor adhesions, which could be making retraction or coverage more difficult."
Next Steps
Clothing Choice: "Wear soft, 100% cotton underwear that is snug but not tight. This prevents the glans from shifting and rubbing against the fabric, which is usually the main trigger for discomfort." Moisturization: "Applying a thin layer of plain, fragrance-free Vaseline (Petroleum Jelly) or a doctor-recommended emollient can act as a protective barrier between the glans and your clothes, significantly reducing irritation." Don't Over-focus: "I understand this is distracting your work, but try not to 'check' or pull the skin repeatedly, as constant friction will only keep the area inflamed and sensitive." Caution: "Avoid using any antiseptic liquids or harsh soaps in that area, as they will strip away natural oils and make the hypersensitivity much worse."
Health Tips

-This appears more consistent with short/tight foreskin with mild chronic irritation or balanitis rather than a dangerous condition -Glans sensitivity and rubbing discomfort can happen when the glans remains exposed for long periods
Next Steps
-Mild exposed glans sensitivity often improves gradually as skin adapts over time -Avoid repeatedly checking or focusing on the area as anxiety can increase discomfort perception -Avoid self-steroid creams without proper advice
Health Tips
-Keep the area clean and dry; avoid harsh soaps and excessive rubbing -Use loose cotton underwear and avoid friction -Continue Pregab Forte only if prescribed and helping symptoms Contact me anytime on practo for further consultation

From the history and image, this does not look like an emergency, and foreskin glans sensitivity issues can be managed properly after a detailed evaluation 👍 Since this has been troubling you for a long time and affecting your comfort and confidence, it’s better to discuss it in a proper consultation so the exact cause and best approach can be assessed clearly.
