I have symptoms like neck stiffness, lower back pain and stiffness, ear pain, pain under the feet, lower head stiffness and pain. Dizziness and also when hit on the head, it gives a hollow sound. Cracking sounds in the neck, when shaking my head. And also, stiffness in the arms. Consulted with ent doctor, there's no issue with my ear. I'm unmarried and bp is 100/60 What could these refer to?

Presenting Symptoms: Neck stiffness, lower back pain, stiffness in arms Ear pain, cracking sounds in neck Head feels hollow on percussion, dizziness Pain under the feet, lower head stiffness BP: 100/60 mmHg (low-normal) 🔍 Differential Considerations (as per Harrison's Manual): 1. Cervical Spondylosis / Disc Disease Common in young females due to poor posture, long hours at screen/desk Explains neck stiffness, cracking sounds, dizziness (cervicogenic vertigo) May radiate symptoms to head, arms, and ears due to nerve root irritation 2. Fibromyalgia Widespread musculoskeletal pain with fatigue, sleep disturbance Often includes neck, back, arm pain, and vague dizziness Seen in young females, sometimes worsened by emotional or physical stress 3. Myofascial Pain Syndrome Trigger points in neck/back muscles Pain referral to ear, head, arms May mimic ENT or spine-related issues 4. Postural Hypotension / Autonomic Dysfunction BP of 100/60 + dizziness may indicate orthostatic symptoms Can co-exist with fatigue syndromes like chronic fatigue or fibromyalgia 5. Vitamin Deficiencies (especially B12, D) Can lead to dizziness, neuropathic symptoms (feet pain, arm stiffness), fatigue 🚫 Less Likely (but considered if red flags appear): Meningitis – no fever or photophobia present Intracranial pathology – unlikely unless neurological signs or persistent vomiting, visual changes ✅ Recommended Steps: 🩺 Clinical Evaluation: Neurological examination – rule out radiculopathy, cerebellar signs Musculoskeletal exam – check for tender points, posture 🧪 Investigations: X-ray or MRI cervical spine (if cervical spondylosis suspected) Serum Vitamin B12, Vitamin D, Calcium, Thyroid Profile ESR/CRP if inflammatory disorder suspected 💊 Management (based on likely diagnosis): Neck exercises, posture correction, ergonomic advice Short course of NSAIDs for pain Physiotherapy for cervical or lumbar stiffness If fibromyalgia suspected → low-dose amitriptyline or pregabalin 📘 References: Harrison’s Manual of Medicine: Chronic pain syndromes, fibromyalgia, and cervical spine pathology

Your symptoms could be due to a viral infection. Even Vitamin deficiency may be the cause. Please seek a proper consultation with a General Physician in your area for a physical examination and further management

Your symptoms could be related to issues in the musculoskeletal or nervous system, and may involve: 1. Cervical Spine Problems (like cervical spondylosis or disc issues): • Can cause neck stiffness, dizziness, cracking sounds, arm stiffness, and head pressure. 2. Fibromyalgia or Chronic Myofascial Pain: • Characterized by widespread muscle stiffness, pain under feet, neck/back stiffness, and fatigue. 3. Early Ankylosing Spondylitis or Other Inflammatory Arthritis: • Especially if you have morning stiffness in the lower back or heel pain. 4. Vitamin Deficiencies (like B12 or D3): • May cause nerve-related symptoms, muscle pain, dizziness, and fatigue. 5. Postural or Nerve Compression Issues: • Poor posture, especially with prolonged sitting, can lead to muscle imbalance, nerve compression, and stiffness. Your normal ENT exam rules out inner ear causes, making spine or systemic issues more likely. The “hollow” sound you perceive when tapping the head is unusual but could be linked to muscular tension or sinus resonance. Recommended tests: • MRI Cervical Spine • X-ray LS Spine/SI joints • Blood tests: Vitamin D, B12, ESR, CRP, HLA-B27 • Thyroid profile Consult: Neurologist, Rheumatologist, or Spine Specialist as next steps.
Next Steps
maintain proper posture, do light neck/back stretches, ensure good sleep, and consider vitamin supplementation if deficient.
