You had diagnosed all. So treat now. Livogen z one at night for 2 months. Inj uprise d60k one per month for 3 months  vit d3  . Thyronorm 50mcg one before breakfast for two months not to stop follow up by md physician. It's a calcium depleting agent so bones become weak to prevent that add Shalcal xt one at night   no separate treatment for esr.
