Need to ask a lot of questions. What is his creatinine level? Is he on dialysis? Is he taking erythropoietin and iron? Is there any history of blood loss such as bleeding piles?

Get Iron profile, Vit B12 and Folate levels, latest KFT including calcium, phosphate, Hemoglobin) Get intact PTH (If not done already in last one year) Review with complete history to nephrologist.
