Baby can be shifted to home only if breastfeeding, gaining weight and free from any illness
Ideally he should be 2.0 Kg
Or at least 1.8 Kg if mother can care.
If financial problem, then shift to government or charitable hospital
If nothing is possible then try breastfeeding or
Expressed breastmilk every 1 or 2 hours (150 to 200 ml per Kg body weight in 24 hours) try breast feeding. If baby can't suck then by spoon.
Keep baby warm by a method called Kangaroo Mother Care. Check details from net.
Vitamin D 800 units daily
Hope for the best

Hi.. your details are incomplete. In photo you have shared, baby is on oxygen support and on tube feeding. Also, 1.2 kg weighed baby can not be managed at home because of many risks. So you need to continue treatment in hospital only.
Next Steps
You can shift baby to nearby trust hospital or government hospital where all treatment is either free or at low cost.
