Hello, I am Dr Romain Rajan, General Physician. Your case has been allotted to me. I understand your concern. You have severe Vitamin D deficiency, long working hours, heavy weight lifting, stair climbing and repetitive bending. This combination commonly leads to lumbar strain, nerve compression and sciatica, which is exactly matching your symptoms of • Lower back pressure • Pain from hip to toe • Inability to bend • Pain increasing over the past ten days This is not a simple Vitamin D pain anymore. It has now become inflamed nerve pain, and going back to the same type of heavy work can worsen your condition significantly. Advices 1 Avoid lifting weights, bending forward, climbing up and down stools until pain settles. These movements can worsen nerve compression. 2 Apply warm compress to lower back twice daily and strictly avoid soft mattresses. 3 Begin gentle back stretches only after pain decreases, not during the acute phase. What to Monitor 1 Increasing numbness down the leg. 2 Weakness while walking or dragging the leg. 3 Sudden worsening of back pain on bending or standing. Management Your doctor has advised an X ray and physiotherapy, and that is correct. You may require • A short course of anti inflammatory medicines • Muscle relaxant • High dose Vitamin D weekly • Complete rest from heavy physical work for a few days • Physiotherapy to reduce nerve pressure Going back to heavy supermarket work right now is not advisable. Activities like lifting, stocking shelves, climbing stools and standing for long hours will increase inflammation and may cause severe sciatica. You need at least a short medical rest period until pain reduces and physiotherapy begins to work. After assessing your Vitamin D treatment, pain medicines and X ray findings, I can guide you with the exact duration of rest you need. When to Consult a Doctor Immediately 1 If your leg becomes weak or numb. 2 If pain shoots down suddenly or you cannot stand properly. 3 If you lose control over urine or bowel movement. Every patient’s situation is unique. For accurate diagnosis and lasting relief, it is important to discuss your symptoms, job activity and medical history in detail. You can reach out to me directly for this case and also connect with me in future for any medical consultation. I am available on Practo or on WhatsApp at 85271646 seven seven for a one on one consultation where you will receive 1 A personalised treatment plan 2 Safe medicine adjustments if required 3 Quick follow ups to track your recovery Take care, wishing you good health.
