"I want to reassure you that what you are experiencing is a medical condition, not a behavioral issue. Itching that intensifies specifically at night (Nocturnal Pruritus) and involves the trunk, buttocks, and genital area is a hallmark symptom of Scabies—a skin infestation caused by tiny mites that become more active when the body is warm under covers. The urge to scratch in such cases is an involuntary physical reflex, and it is important to treat the underlying infestation rather than feeling embarrassed about the act of scratching."

Hello,
It is important to address this issue clinically, as itching that occurs primarily at night-especially when it involves both the trunk and the genital/buttock regions—often points to specific dermatological or environmental causes rather than a lack of hygiene.
1. Potential Cause: Scabies
The most common cause of intense itching that worsens significantly at night is Scabies, a skin infestation caused by tiny mites.
Key Indicators: It often affects "thin-skinned" areas, including the finger webs, wrists, armpits, and significantly, the genital and buttock regions.
Nocturnal Pattern: The itching is famously worse at night because the mites become more active when the body is warm under covers.
2. Other Considerations
Nocturnal Pruritus: Sometimes, itching is linked to the body's natural circadian rhythm, where inflammatory mediators increase at night while anti-inflammatory corticosteroids are at their lowest
.
Contact Dermatitis: Sensitivity to laundry detergents, fabric softeners, or the material of your nightwear/bedding can cause irritation that you only notice once settled for the evening.
Immediate Recommendations
Physical Examination: You must visit a Dermatologist. They will look for "burrows" (tiny raised lines on the skin) or small red bumps that confirm a mite infestation.
Avoid Self-Medication: Do not apply strong steroid creams or antifungal powders until a diagnosis is made, as these can sometimes mask the symptoms of an infestation and make it harder to treat
.
Hygiene Measures:
Wash all bedding, towels, and recently worn clothes in hot water and dry them on a high-heat setting to kill any potential allergens or mites.
Cut your fingernails short to prevent skin infections from scratching during the night.
A Note on the Incident with Your Mother
It is completely understandable to feel embarrassed about being seen in a vulnerable moment. Explaining to her that you have a persistent medical condition causing this intense discomfort may help ease the tension and ensure you get the medical care you need.
Summary: Nighttime itching that involves the private areas is a hallmark of Scabies, which is a common and treatable medical condition. A dermatologist can provide the correct medicated cream (usually Permethrin) to resolve this quickly.
Best regards.
