My issue that I try to do insert Penis in Vagina with out condom i think it's not gone inside and penis touch vagina and vagina fluids I think so i dont know and immediately I wear condom in penis and have done exposure and after that I check no broken no slip condom & there is minor penis head skin break

See after exposure it takes months to years for hiv to produce symptoms , so in no way hiv is causing headache. It's just your anxiety related to it . Feel free to ask if you got any doubt. Just take full dose of prophylaxis medicine for 28d as there was contact with vaginal fluid so we can't say you are absolutely free from consequences yet but risk is very low for you thats best news . Get hiv test after 1 and half months for reassurance.
You still need to take care of few things cause it's not just hiv .. syphilis gonnroehea other stds ..we need to give you complete std regimen.. for that we need you to conduct few other blood tests cause these stds can be transmitted to other person including your wife so we need to cure it before that happens. Also if you had sex with other people after sex with commercial worker we need to give them same prophylaxis.
