As pain is since 2-3weeks and you are on medication which causes constipation, which is the reason for fissure formation. You can try conservative management.
1. Avoid spicy food, take fibre rich food and adequate hydration
2. Oint Anobliss to be applied in and around anal region 5-10min before defecation
3). After defecation, sitz bath.
4) Syp cremaffin / stool softener daily at bed time

If your medication is continue for long time then you can think about surgery bcuz this medication leads to constipation and conspation leads to fissure. So try to avoid constipation before going for surgery. surgery is beneficial if u don’t tolerate and your problem not solve with medication Kindly mention what type of knee surgery and which antidepressants you are taking.
