Hair fall, scalp pain and hair thinning

Right from April end, I'm facing hair fall that is moderate to severe. Even though I use a wide tooth comb, the hair fall remains the same. My scalp is very sore and painful, though it's not red( it occassionally itches). I had thicker hair before, but now it has gotten thinner. My forehead hair and crown area has opened up more.
